USBC ADULT MEMBERSHIP APPLICATION

When filling out the sanction cards please make sure that your bowlers fill in all sections. Sanction Cards will be mailed directly to the bowler so it is very important that their current address is on the cards. You may receive some of the cards preprinted with the bowler information on it, make sure that the address is current.

Membership choices:

  1. Standard: A bowler that is paying their sanction fees through your league. Fee = $19.00
  2. Local Only: A bowler who is sanctioned in another association, example Westchester. Fee = $8.00
  3. None: A bowler who is in your league but paid their sanction fee in another league this season. They must enter the Name of the League and Center in which they paid. Fee = $0

Senior Membership choices:

  1. Standard: A bowler that is paying their sanction fees through your league. Fee = $15.00
  2. Local Only: A bowler who is sanctioned in another association, example Westchester. Fee = $4.00
  3. None: A bowler who is in your league but paid their sanction fee in another league this season. They must enter the Name of the League and Center in which they paid. Fee = $0

Women's State Membership choices:

  1. USBC State WBA: Join the Women's State Association. This is the default for women. Additional Fee = $0
  2. USBC State BA: Join the Men's State Association. Additional Fee = $0
  3. Both: Join the Women's State Association and the Men's State Association. Additional Fee = $1 (Total sanction fee =$20.00 ($19 + $1))
